What is gynecomastia?
Gynecomastia is enlargement of glandular breast tissue in men. It can affect one or both sides and may occur alongside excess chest fat or loose skin. Hormonal changes, medicines and some medical conditions can contribute, so assessment comes before a treatment decision.
Breast enlargement during puberty may settle over time, while persistent enlargement may lead someone to seek treatment. Male breast reduction aims to improve chest contour using liposuction, tissue excision or a combination, according to the tissues involved.

What your assessment covers
Discuss when the enlargement began, whether it has changed, any discomfort and the impact on daily life. Your medical history, medicines and supplements help identify possible contributing factors. Do not stop prescribed treatment without advice.
The examination looks at the amount and distribution of glandular tissue and fat, the skin's ability to retract, nipple position and differences between the two sides. Any unexplained lump or new nipple change needs medical assessment. Age alone does not determine suitability; in younger patients, development and the possibility of recurrence also matter.
Treatment options
Approaches to male breast reduction
The incision and technique depend on the volume of gland, fat and loose skin. The following approaches may be considered individually or in combination.
Excision through the areolar border
An incision along part of the lower edge of the areola can provide access to remove excess glandular tissue. The amount removed is planned to improve contour while reducing the risk of a hollow or uneven appearance.
Skin reduction and nipple repositioning
More marked enlargement or loose skin may require removal of skin as well as breast tissue. The nipple and areola may need repositioning or reduction. This involves a different scar pattern from liposuction alone.
Pull-through technique
In selected cases, glandular tissue is surgically released and removed through a small access incision, often one used for liposuction. The gland is excised; it is not simply suctioned out as fat. Suitability depends on the tissue and skin.
Ultrasound-assisted liposuction
Ultrasound-assisted liposuction is one method used to treat the fatty component of an enlarged chest. Firm glandular tissue or substantial excess skin may still need excision. The choice of liposuction method does not eliminate surgical risks or scars.
The procedure
Liposuction, excision or a combined approach
Where excess fat predominates, liposuction removes fat through small incisions using a cannula. Excision is used when glandular tissue or excess skin needs removal. Many surgical plans combine the two to address the chest as a whole.
The anaesthetic is chosen for the extent of treatment and your health, and may involve general anaesthesia or an appropriate sedation/local anaesthetic plan. Surgery often takes around one to two hours, but more extensive treatment can take longer. You will be observed afterwards and given discharge and follow-up instructions.
Ask about incision placement, any drains or compression garment and how much correction is realistic. Bleeding, fluid collections, infection, uneven contour, altered nipple sensation, healing problems and visible scars are among the risks.
After your procedure
Recovery and settling of the chest contour
Soreness, bruising and swelling are expected early in recovery and can temporarily obscure the contour. Prescribed pain relief and a support garment may help. If drains are used, their care and removal are guided by the surgical team.
Some people can return to light work around a week after limited treatment, while more extensive tissue or skin removal can require longer. Follow individual advice on driving, lifting and exercise rather than using work return as a sign that healing is complete.
The chest continues to settle over the following months; the final result may take three to six months to assess. Scars are permanent, although they usually fade. Results vary, and further treatment may be needed for a residual contour concern or a complication.
